NDIS Worker Screening Register Template

What an NDIS worker screening register should track for risk-assessed roles, clearance status, expiry dates, evidence and supervision controls.

Why worker screening needs its own register

Worker screening is one of the easiest areas for a provider to under-control because expiry dates, role changes and contractor arrangements move constantly. A single register helps the provider prove who is in a risk-assessed role and whether the right clearance evidence exists.

The register should not sit in isolation. It should connect to onboarding, training, competency, supervision, incidents and audit evidence.

Core fields to include

A practical register should be simple enough for operations teams to keep current, but structured enough to satisfy audit and governance review.

  • Worker name, role, employment type, location and supervisor.
  • Risk-assessed role status and assessment date.
  • Clearance number, clearance status, verification date and expiry date.
  • Evidence file and source of verification.
  • Supervision or management plan where required.
  • Induction, Code of Conduct acknowledgement and mandatory training status.
  • Alert date, owner and next review date.

Controls that make the register audit-ready

The register becomes valuable when it drives action. Expiry alerts, missing evidence flags, owner assignment and review notes show that the provider actively monitors workforce safeguards.

  • Flag expired or missing clearances before workers are rostered into risk-assessed roles.
  • Connect clearance evidence to the worker profile.
  • Track role changes that create a new screening requirement.
  • Record supervision controls for workers awaiting clearance where permitted.
  • Review the register before internal audits and registration renewal.
